11.2.1 install issue (Mac)

Anyone seen this issue when installing the upgrade?



Could not create a new document because Assertion in "/Volumes/Development/139084/ProTools/DFW/MacBuild/../Views/UMenu.MacOS.mm", line 1549.

I am thinking that it is a 10.9.4 issue?



correct driver has been installed.
