License transferability PT11HD versus PT12HD?

I am reopening this closed thread because Avid support has not addressed my specific questions. Most specifically, the link that JC sent me to clearly states:

"Please note that Pro Tools software products that have been upgraded to the newer 2015 support plans and newer Pro Tools products which include 2015 support plans are not currently available for transfer."

I walked through the form and was able to define my product as DAW | Pro Tools HD | Omni, with no mention of software. Would this be a turnkey Transfer of Ownership that includes Omni + PCI Card + the original PT10HD license that shipped with the product, or my current updated PT11HD license, or no software at all? If the latter, how does the buyer secure software for the system, the answer to which would affect my sellability? Finally, does the Avid fee include the PACE charges, or would that be an additional fee?

I have not found specific answers to these questions in the FAQ or forms sent to me as links, and want to have answers before proceeding with submitting the form.
